Drinking Water Quality Reports (Consumer Confidence Reports) are mandatory reports that tell you the customer about the
water you drink. There are two reports - one for the North which is our Brookshire Water Plant services; and one for
the South which is our McKay Water Plant Services.

Final Bill Request Form
If you are moving away from the District and need to have service de-activated, fill out this form and send it to our
administrative office. We will turn off service on the day specified, final bill, and send the balance left from the
deposit to the address requested.

Notice of Requirement to Comply with the Subdivision & Service Extension Policy
This handout explains what the Districts responsibility and the Consumers responsiblity is for providing service
to an area.
Payment Plan Agreement
If you can't pay your water bill by the termination date, you can fill out this Payment Plan Agreement form and turn
in to the Administrative office. This form will give you a little extra time to pay your bill and helps to avoid loss
of service and reconnect fees. The District only allows TWO arrangements in a twelve month period. This form must
be filled out before services are turned off.
Public Info Request
If you would like to request public information, fill out this form and return to the Office Manager at the administrative
office.
Real estate inspection 72hr
Fill out this form if you are a real estate broker and would like to have water service turned on for 72-hours for an
inspection.
Request to Appear before the Board
If you would like to speak at our monthly board meeting, fill out this form and turn it in to the Office Manager by
12:00pm on the second Wednesday of the month.
